What is Personalised Care and Support Planning?

Personalised care and support planning is a series of conversations between a service user who requires care, their family, partners or carer(s) or health and social care professionals.
The aim of these conversations is to enable the individual to be an active participant in managing their health and wellbeing, taking into account all parts of a person’s life to ensure they have a role in their own care and a say in how they are cared for.

What Does Personalised Care and Support Planning Mean for Service Users?

A care and support plan puts the ‘person at the centre’, allowing the service user greater choice and control over how they are cared for and agreement of the mechanisms that are put in place to achieve this.
Through this, the person is able to lead a more fulfilling life, not missing out on things that a less rigorous system of care might overlook.
Care and support planning enables person-centred and coordinated care.

Who May Need a Care and Support Plan?

A care and support plan may be needed by any person who requires support from health and care services over time and who has a health condition which can be improved or managed through person-centred approaches.
People who might need a care and support plan may include:
  • a person with one or more long-term health conditions (for example, arthritis, asthma, COPD, depression, dementia, diabetes and many more)
  • a person with a disability
  • a person who is recovering from illness or other problems
  • a person who is receiving end of life care
  • an older person who is frail or is at risk of becoming frail
  • a person with complex needs who is receiving care and support from a number of different agencies - for example, a person with substance misuse problems.

What Does a Care and Support plan Include?

The aim of the planning conversation is to find out things about the person that will help inform their care and support and be included in their care plans.
This can include, finding out more about the person including likes and dislikes, friends and family and discussions about what support they need.

What are the Benefits of Personalised Care and Support Planning?

When people are given the opportunity to actively participate in the planning and management of their health and are valued as experts in their own wellbeing, the outcomes and solutions developed have meaning to the person in the context of their whole life. This leads to improved chances of healthcare professionals successfully supporting them.

What Does This Personalised Care and Support Planning eLearning Course Cover?

This course is a guide to the care and support planning process, covering how care and support planning is beneficial to service users who require ongoing support from health and care services, particularly those with health conditions that can be managed or improved through person-centred approaches. It details four steps of the care and support planning process: prepare, discuss, document, and review. The course is aimed at health and care professionals who have regular contact with service users and who may be involved in the care and support planning process, and is available now on our GP, Private & Urgent Care, Hospice and Health & Social Care platforms.
